We are testing new ways to identify and engage with people who have, or are at risk of, long-term health conditions and who need financial support.

We are piloting new ‘healthcare pathways’ to support residents in Lambeth and Southwark. Through to the end of September 2025, we expect to provide support to over 2,000 residents in these areas.

Identifying patients in need of financial support

We are working with over 20 GP practices in Dulwich, Stockwell, Peckham, South Bermondsey, and Walworth. GP practices identify working-age patients with, or at risk of, long-term health conditions and proactively message these to offer support. Practices also make referrals via their social prescribers, and display posters and leaflets directing patients to our dedicated ‘Back on Track’ website where they can request an appointment. And, of course, GPs can make direct referrals themselves too.
